- $125,000 to $170,000 Annual Income
- $105,000 to $125,000 Base Salary

This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 25 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.
POSITION DESCRIPTION:
The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients. Most patients are located in private homes (house calls), with a limited number in long-term care facilities (SNFs, ALFs, ILFs). Over time, the patient base will transition to more facility-based days and fewer house calls.
Providers see a variable number of patients per day, dependent upon type of visit and actual travel time. Dedicated schedulers put together daily schedules and routes for the providers to ensure efficiency.
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and adjust treatment plans as necessary
- administer treatments and interventions as appropriate
- perform minor procedures and wound care as required (and settings permit)
- monitor vital signs and patient responses to treatment
- educate patients and families on health management, disease prevention, and treatments
- provide guidance on lifestyle modifications and self-care practices
- work with physicians, nurses, and other professionals to coordinate comprehensive care
The practice uses the eClinicalWorks EHR.
On-call is phone triage only with very low volume. Providers rotate the on-call responsibility, with a shift every 12 weeks or less.
REQUIREMENTS:
- Ohio NP license
- DEA
- advanced practice experience preferred, not required
- new grads with RN backgrounds in home health, trauma, or ICU encouraged to apply
