SCHEDULE:

COMPENSATION:
- $125,000 to $155,000 Annual Income
- $105,000 to $125,000 Base Salary
- $20,000 to $30,000 per Year in RVU Bonuses

This practice sees patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), and private homes.

This organization is dedicated to delivering convenient, high-quality primary and psychiatric care to geriatric patients in the comfort of where they call home.  They currently boast a team of more than 15 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office.  Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.


POSITION DESCRIPTION: 
The psychiatric mental health nurse practitioner (PMHNP) will work with a dedicated panel of geriatric patients.  All patients are located in facilities or private homes.

Daily volume varies based upon types of appointments scheduled, patient needs, and number of locations being visited.  The PMHNP will have a dedicated scheduling team member who creates an efficient schedule for each work day.

Specific duties of the role include, but are not limited to:

- psychiatric wellness assessments and care
- prescribing age-specific physical therapy and rehabilitation
- prescribing pharmacologic treatments
- diagnosing, treating, and helping patients manage acute and chronic illnesses
- conducting examinations and interpreting medical history
- ordering and performing diagnostic tests and/or procedures, when appropriate
- working with physicians, nurses, and other professionals to coordinate comprehensive care
- ensuring compliance with all relevant healthcare regulations and standards

The practice uses the eClinicalWorks EHR.



REQUIREMENTS:
- Ohio NP license
- DEA
- PMHNP certification
- advanced practice experience preferred, not required
