- $180,000 to $225,000 Base Salary, depending on experience

COMPANY PROFILE: 
This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home.  They currently boast a team of more than 20 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office.  Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.


POSITION DESCRIPTION: 
The physician will serve as the primary care provider for a dedicated panel of geriatric patients.  

All patients are located in facilities or private homes.  Depending on the territory, the practice is able to offer different mixes of facilities (ALFs / ILFs / SNFs) and home-based patients.  During the initial phone interview, the practice will look at the physician’s home location and discuss this in detail, mutually agreeing upon a geographic territory and patient settings.

In SNF settings (with no travel during the day - only one location per day), physicians see an average of 15 patients per day.  

If traveling between ALFs, ILFs, and homes, physicians see an average of 9 to 12 patients per day.

Specific duties of the role include, but are not limited to:

- conduct health assessments to identify patient needs
- evaluate patient conditions and adjust treatment plans as necessary
- perform minor procedures and wound care as required (and as settings permit)
- educate patients and families on health management, disease prevention, and treatments
- work with other providers as needed to coordinate comprehensive care

The practice uses the eClinicalWorks EHR.



REQUIREMENTS:
- Ohio medical license
- DEA
- BC/BE status
- no experience required
